Debbie is , our new Primary Care Navigator for the Turton Neighbourhood. Debbie provides all kinds of support for the people in Turton. Here is what Debbie had to say about her role.

"I am thrilled to be working in the Turton Neighbourhood in my new role as Primary Care Navigator. I am able to provide personalised support to the elderly, vulnerable and socially isolated with...in the community. My aim is to get to know the individual and their family/carers and help to improve their health and general wellbeing by supporting them in navigating the health and social care systems.

I can signpost to community services and activities and provide help, guidance and support in many ways. I am able to give the time to listen and look at solutions or ways to make day to day life a little better.

I have lived and worked in the area for many years and look forward to being able to give something back into our local community."

District Nurses

Health Visitor

The practice has an attached Health visitor and is available to discuss health problems and runs the baby clinic.

Midwife

Is responsible for providing your ante-natal care, delivering your baby and caring for you post-natally.

Health Trainer

NHS Health Trainers have been specifically trained to motivate, support and advise people on how to get healthy, stay healthy and get the most out of life. They encourage people to change lifestyle behaviours which are known to cause ill health.

Our Health Trainers are based within GP Surgeries across the Borough. They specifically work with patients who are 45 years and over, who do not have any existing cardiovascular conditions such diabetes and heart disease.

Although you may be well at the moment, there are likely to be aspects of your lifestyle that could prevent you from staying well. If you smoke, do little or no physical activity, drink more than the recommended limits of alcohol or eat unhealthily then you could be putting yourself at risk of becoming ill in the future.

Small changes can make a big difference to people’s health and that is what Health Trainers are about. They will offer motivation and encouragement, and will help people to find and access the support and services they need.

Bolton’s Health Trainers are passionate about making a difference to people’s health and wellbeing.

LGBT Foundation

Our practice has recently been working with the LGBT Foundation as part of the Pride in Practice training, for any of our patients who identify as lesbian, gay, bisexual or transgender.  The LGBT Foundation, which is based in Manchester, can provide a wide range of help and services for people who feel they might need some additional support.  Some of the services they offer include:

  • Counselling
  • Outreach
  • Pop-in support
  • Sexual Health Testing
  • Support Groups
